Why put dates on it at all
Because a plan expressed in weeks post-op is impossible to reason about against a real calendar. Week 36 means nothing. A date in the middle of next season means a lot, and it is the sort of thing worth knowing in month two rather than month seven.
Two things follow from seeing the real dates. Athletes with a season on the line find out early whether the nine-month mark lands before, during or after it. And everyone else stops treating month six as the end, because there is visibly a lot of calendar after it.
What the dates are and are not
Dates are arithmetic, not permission
Every milestone in this list is gated by criteria rather than by the calendar. Week 16 is when running usually becomes possible; it becomes possible when the knee is quiet, the extension is full and the quadriceps is strong enough, and if those are not there, the date is irrelevant. The 2017 clinical practice guideline recommends criterion-based progression precisely for this reason.
The nine-month mark, and why it is on the list
In the Delaware-Oslo cohort of 106 pivoting-sport athletes, the reinjury rate fell 51% for each month return to sport was delayed, up to 9 months, with no further reduction after that. In a Swedish registry study of 159 athletes aged 15 to 30, returning to knee-strenuous sport before 9 months was associated with a hazard ratio of 6.7 for a second ACL injury. Neither study says month nine is a switch. Both say the months before it are the expensive ones.
Then keep going
The last milestone in the list is year two, and it is there because that is where the risk sits. In one series of 316 patients, graft ruptures happened on average 1.8 years after surgery, with 47% in the first postoperative year and 74% within two years. The people who stop training the week they get cleared are training for the wrong finish line.
