Limb symmetry index calculator

The unit almost every ACL return criterion is written in, the thresholds that go with it, and the arithmetic trap that makes a weak leg look ready.

Limb symmetry index

Operated leg divided by other leg, times 100. Use the same test, the same day, the same warm-up, and the same units on both sides. Newton-meters, pounds, kilograms, centimeters or seconds all work as long as both boxes use the same one.

The trap in every symmetry number

A ratio has two sides

Limb symmetry index compares your operated leg to your other leg, and your other leg has been through nine months of reduced training too. If it loses 10% while the operated leg gains nothing, your symmetry improves and your knee is no better. This is why absolute numbers should be recorded alongside the ratio, and why the other leg is trained like a leg at risk rather than treated as a fixed ruler.

There is a second reason to train the other side: in a meta-analysis of 19 studies, contralateral ACL injuries (8%) slightly outnumbered graft ruptures (7%), and in one cohort followed for two years after return to sport, 20.5% injured the other knee against 9.0% who retore the graft.

What to measure it on

  • Quadriceps peak torque on an isokinetic dynamometer, which is what the literature uses, or isometric force on a handheld dynamometer, which is what most clinics have.
  • Hamstring strength, especially after a hamstring autograft, where a lower hamstring to quadriceps ratio at 60 degrees per second was associated with graft rupture in a professional cohort.
  • Single hop, triple hop, crossover hop for distance, in centimeters or inches.
  • Six-meter timed hop, in seconds. Remember the ratio inverts for a time: faster is a smaller number, so divide the other way round or you will read it backwards.
  • Single-leg press or extension load, tracked with the same setup. Not comparable to a published threshold, and perfectly good for direction.

Why this one number matters so much

More symmetrical quadriceps strength before return significantly reduced the reinjury rate in the Delaware-Oslo cohort. Athletes who returned to sport with quadriceps asymmetry were still unloading the operated leg two years later, visible in knee flexion excursion and peak vertical ground reaction force. The deficit does not quietly resolve.

And the honest counterpoint, which belongs here: in a Swedish registry cohort of 159 athletes, symmetrical muscle function and quadriceps strength at return were not associated with a second ACL injury, while returning before nine months was. Symmetry is a goal worth chasing. It is not a substitute for time.

Questions people actually ask

What is a good limb symmetry index after ACL surgery?
Ninety percent or better is the threshold used in most return-to-sport batteries. At week 12, 60 to 80% is normal. At month nine, under 85% means the gap is likely to still be there at month eighteen unless the program changes.
How do I calculate LSI for a timed test?
For a time, faster is better, so the ratio inverts: divide the other leg's time by the operated leg's time. If you put the times in the same order as a strength measurement, a slower operated leg will read as above 100%.
Can my symmetry index be wrong?
It can be misleading in two common ways: the other leg has deconditioned, which inflates the ratio, or the test allows a compensation pattern, which is easier on hop tests than on a dynamometer. Record both absolute values, not only the ratio.
Do I need a dynamometer?
For a number comparable to the published thresholds, yes, and most physical therapy clinics have at least a handheld one. For tracking your own direction month to month, a consistent single-leg press setup is genuinely useful.
