How long does ACL surgery recovery take?

Short answers to the questions people actually ask, each one with what it depends on: walking, driving, work, school, running, lifting, and going back to your sport.

The question always arrives as one question and it is really eight. Recovery to what? Walking through an airport, sitting through a shift, jogging three miles, and cutting at full speed in a game are four different finish lines, months apart. Here they are separated, with the thing that actually decides each one.

Why the sport answer is a range and not a number

Because the two things that predict a second injury pull against each other. Time since surgery is protective: reinjury fell 51% for each month return was delayed up to 9 months in the Delaware-Oslo cohort, and returning before 9 months carried a hazard ratio of 6.7 in a Swedish registry study. Passing a criteria battery is also associated with lower risk in some cohorts: 38.2% of athletes who failed were reinjured versus 5.6% of those who passed.

But a 2019 meta-analysis of four studies found the pooled risk difference for passing criteria was -3%, not statistically significant, at very low quality of evidence, and 14.4% of those who passed still sustained a second ACL injury. So the honest position is that time and testing together are the best available guide, and neither is a guarantee.

Age changes the arithmetic more than anything else

If you are 17 and going back to soccer, your risk profile is not in the same universe as a 38-year-old going back to recreational tennis. In a meta-analysis of 19 studies, the second ACL injury rate was 15% overall, 21% under age 25, and 23% in athletes under 25 who returned to sport, which is roughly one in four and 30 to 40 times the risk of an uninjured adolescent. The highest graft rupture rate in one 316-patient series, 28.3%, was in males under 18. In the MOON cohort, the odds of graft rupture rose 2.3 times for every 10-year decrease in age.

What actually delays people

Not motivation. In practice it is a returning effusion, an extension deficit that was not fixed in month one, a quadriceps that was never measured, a meniscus repair with its own restrictions, and insurance running out of authorized physical therapy visits. The last one is real and it is plannable.

What returning to sport actually looks like

Nobody goes from cleared to a full game. The progression is graded and it is the part most often skipped: full practice without contact, then controlled contact, then full practice, then limited minutes, then unrestricted. In the pooled data, 81% return to some form of sport, 65% to their preinjury level, and 55% to competitive sport. The gap between 81% and 55% is mostly not surgical.

Questions people actually ask

Can anyone return to sport at 6 months?
Some do, and it is where the risk concentrates. In one professional cohort, graft ruptures happened on average 105 days after return, and returning without meeting all six discharge criteria carried a hazard ratio of 4.1. Six months with a fully passed criteria battery in a low-risk sport is a different decision from six months in high school football.
How long am I on crutches?
Usually days to about three weeks. The gate is a normal walking pattern without them. Keeping crutches longer than needed slows the quadriceps down; dropping them while you still limp trains the limp.
When does the swelling finally stop?
Post-surgical effusion typically settles over four to eight weeks, but the swelling that matters is the swelling that comes back after activity. That one can show up at month six and it is information: the load exceeded what the leg can currently absorb.
Do I need the full 9 months if I only want to ski or lift?
The 9-month figures come from cohorts returning to pivoting and knee-strenuous sport. Straight-line running, cycling and lifting come back much earlier and carry far less reinjury exposure. Skiing sits in between, and the honest variable is what happens when you fall, not what happens when you turn.
